Last year, nearly 30,000 people turned to us for help to address a broad range of challenges. We are looking for qualified candidates with experience in case management, community resources, mental health and drug and alcohol outpatient treatment to serve families involved with Child Welfare, the homeless, persons with HIV/AIDS and individuals with co-occurring mental illness and substance abuse disorders.

Open Positions

Lead Resident Life Associate

Full time

Job Summary: Assist the Shelter Operations Manager in administering ongoing operations and activities of the Shelter. Responsibilities include staff guidance and all aspects of client support.

Education: High School diploma or equivalent required. Undergraduate degree in a relevant field preferred. Must have at least 2 years’ experience in a residential facility.

Experience: Experience serving clients with characteristics similar to those of the shelter population is preferred.

 

Parent Educator (Langhorne)

Part-time

Job Summary: The Parent Educator case manager engages the service consumer (client) of an agency program or service in designing and following a goal-oriented plan that will respond to his/her primarily to their parenting skills, knowledge and abilities, without neglecting the client’s physical and/or behavioral health, social and/or environmental needs as identified through formal assessment or reassessment interviews. The Parent Educator case manager empowers the consumer to maximize use of personal and informal resources, links the consumer to community and formal resources as needed and assures that resources are the least restrictive and least intrusive possible. The Parent Educator case manager acts to ensure that the client obtains timely access to needed services, appropriate levels of care and continuity of care.

Education: Bachelor of Social Work degree –from a school of social work accredited by the Council on Social Work Education (preferred) OR Bachelor’s degree – in a closely related health or human services profession from an accredited graduate program. Knowledge of child and family service systems. Valid Driver’s License (issued by state in which employee lives) and safe driving record and proof of insurance coverage. Knowledge of social work values and/or child welfare services. Experience with computers and web-based systems.

Work/Internship Experience: Two or more post-graduate years of successful employment (preferred) or internships. Two years case management experience involving child and family services (preferred). The work experience should reflect service to clients with characteristics similar to the specific population target by the Nurturing Parenting program. Comprehensive knowledge of community-wide health and human service systems and facility in differentiating the specific resources appropriate for the problems commonly encountered by the client population serviced in this program and position. The work experience should indicate increasing levels of responsibility, independence and proactive advocacy for and with clients.

 

School Based Outpatient Therapist 

Fulltime

Job Summary: Family Service is seeking an experienced LPC, LSW, LCSW or licensed-eligible clinician for a full-time position in the Langhorne area. The ideal candidate will have experience working with children, adolescents, and adults. The clinician will provide services to students in a school-based environment as well as in the outpatient program of Family Service. Substance Abuse experience a plus. Must be willing to work flexible hours.

Education: Master of Social Work degree from a school of social work accredited by the Council on Social Work Education or a Master’s degree in a closely related health or human service profession from an accredited graduated program.

Experience: One year or more of clinical experience or post-graduated years of successful employment or internships preferably serving clients in the characteristics similar to those of the specific populations targeted by the agency program, project or service covered by this position. Prerequisite job skills and aptitude for on-the job training may be demonstrated by documentation of successful prior employment or internships. Good writing skills, ability to handle crisis situations. Comprehensive knowledge of community -wide health and human service systems and facility in differentiating the specific resources appropriate for the problems commonly encountered by the client population served by the agency program, project or service covered by this position is preferred.

Other Credentials: The agency may require specific credentials or documented training/ experience for specific assignments covered by this job Classification &Title. The Letter of Hiring or an appendix to this position description will detail such credentials, e.g. training/experience in group work, substance abuse intervention and treatment; Pennsylvania operator’s license, current and valid; ownership or use of an insured automobile; health certification; police clearance for working with children.

HIV Medical Case Manager

Full time

Job Summary: The Medical Case Manager engages clients of FSABC’s  HIV/AIDS Program in designing and following a goal-oriented plan that responds to his/her health (physical and emotional), social, and environmental needs identified through formal assessment and re-assessment interviews.

Education: Bachelor of Social Work from a school accredited by the Council on Social Work Education or Bachelor’s degree in a closely related health or human service profession from an accredited college or university.

Experience: Two or more years of successful employment in direct service practice at the bachelor’s level and including case management experience specifically relevant to this position and serving clients with characteristics similar to those of the specific population targeted by the agency program, project, or service covered by this position.  Comprehensive knowledge of community-wide health and human service systems and facility in identifying resources appropriate to the kinds of problems commonly encountered by the client population served by this position.

 

Community Health Nurse

Fulltime

Job Summary: The Community Health Nurse will provide mobile outreach, assessment, and minor treatment to homeless individuals in the community. Services are mobile in order to reach more people. They may be offered inside a dedicated van, at a homeless encampment, within an emergency shelter, drop-in centers, etc., depending on the individual’s choice. The nurse will use the Nurse Process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation to identify physical, mental health, and substance use concernsThe nurse will provide intervention for and/or treatment of health concerns based on the assessment and individual feedback. On-site treatment of minor medical concerns, such as wound care, will be provided to individuals as needed. Assessment for substance use and linkage to evidence-based treatment, with emphasis on Medication Assisted Treatment (MAT,) will be a core component to this service. The nurse assigned to this will take a trauma-informed approach, support harm reduction, and use motivational interviewing to develop trust, provide treatment, develop goals, and support people in reaching these goals. The nurse will provide ongoing outreach and engagement and will connect patients to a primary or specialty care provider when a patient is ready for that next step in care. The nurse will work in partnership with mental health, substance use, case management, street outreach, and other care team members to support patients in maintaining their health and, eventually, their housing.

Education: Licensed Practical Nurse (LPN); Registered Nurse (RN), or Certified Registered Nurse Practitioner (CRNP). If CRNP, DATA 2000 waiver is required.

Experience/Knowledge: Minimum two years of clinical experience with an emphasis on community health. Enjoys working at the top of their scope of practice and collaborating with a diverse team. Enjoys working with individuals who suffer from substance use disorder, mental illness, and who are medically complex. Understanding of, passion for, and commitment to the guiding principles of harm reduction. Understanding of, and passion for, the concerns and principles of public/population health. Bi-lingual (English/Spanish) highly desirable. Must be able to perform basic health screenings using point-of-care testing equipment and assist with transport of supplies to and from community site. Continuing education, annual health assessment, fit testing, vaccinations, and community health education as directed.

Other credentials: Candidates with CRS, CFRS or CPS certification are welcome. Family Service may require specific credentials or documented training/experience for specific assignments covered by this Job Classification & Title. This position requires:

o Training/experience in working with individuals experiencing homelessness

o Valid Driver’s License (issued by state in which employee lives)

Links Case Manager

Knowledge of child and family service systems. Valid Driver’s License (issued by state in which employee lives) and safe driving record and proof of insurance coverage. Knowledge of social work values and/or child welfare services. Experience with computers and web-based systems.

Experience: Two or more post-graduate years of successful employment (preferred) or internships. Two years case management experience involving child and family services (preferred). The work experience should reflect service to clients with characteristics similar to the specific population target by the Nurturing Parenting program (Families with a high risk of abuse and/or neglect. Comprehensive knowledge of community-wide health and human service systems and facility in differentiating the specific resources appropriate for the problems commonly encountered by the client population in LINKS Family Reunification serviced in this program and position. The work experience should indicate increasing levels of responsibility, independence and proactive advocacy for and with clients.

Internships

Social Work Interns

Interns in our Outpatient Behavioral Health program must be master’s level students enrolled in a graduate social work program or in a related master’s level program such as counseling or counseling psychology. We do not accept practicum students. Our Case Management programs accept first year graduate social work students. Interns must be able to work 2 to 3 days per week for the entire academic year (i.e. September through early May).
